I am requesting to demonstrate proficiency (77%) in the approved standards, benchmarks, and skills in a course at Hudsonville High School. I understand that this assessment may include a comprehensive examination, written reports, research papers, a portfolio, or other assessments required when taking the course. I can only earn a credit notation that will not be used in computing my grade point average (GPA) but will count for graduation requirements. I am aware that if I earn a credit notation in this course, I cannot subsequently request an individual assessment for a prior course in the sequence or enroll in a lower course in the same subject sequence. I also understand that there is a non-refundable $20 fee for the test and study materials.

Exams will be proctored between July 28 and August 1. Materials and details will be dispersed to students during the last week of school.
